PATIENT GUIDELINES FOR MEDICAL RECORD REQUESTS
South Texas Spinal Clinic has partnered with HealthMark Group to handle our release of information process, encompassing medical records and billing.
HealthMark Group provides personal copies of records to patients at no cost.
To initiate a request, please follow these steps:
In Office: Request a Medical Release Form from the front office staff. You can complete the form in-office, and we will forward the request to HealthMark. Alternatively, you can fax the completed form directly to HealthMark Group at Fax num. 800-833-5935.
Online: Submit a request for records directly on Healthmark’s online Portal. Create an account to send and receive electronic copies of requested records.

How long will it take to obtain my records? While the Texas State Board of Medical Examiners allows 15 business days for record release, we strive to fulfill requests within 3-7 business days, in accordance with our policy.
Are all my records available? We provide records created by our office, including x-rays and relevant tests ordered by our physicians. Records from other healthcare providers need to be obtained separately.
What if my records are from several years ago? Medical records are maintained for a minimum of seven years. Retrieving archived records may take additional time, and we appreciate your patience. We aim to fulfill requests within 10 working days in such cases.
Is a release for medical records form necessary? Yes, completion of a release for medical records form is necessary as per HIPAA requirements. Pick up a copy at our office, and we will forward it to HealthMark for processing. Alternatively, fax the release directly to HealthMark at Fax num. 800-833-5935. If my child is over the age of 18, am I entitled to the medical records? Individuals over 18 are considered adults. If your child listed your name on the HIPAA form, you are entitled to obtain their medical records.
